This Privacy Policy describes how Accord Specialty Pharmacy collects, uses, and protects information gathered through this website. It applies to accordspecialtypharmacy.com and does not govern the clinical handling of patient health information, which is addressed in our separate HIPAA Notice of Privacy Practices.

When you use a contact form or referral form on this site, you may provide your name, email address, phone number, and the content of your message or inquiry. Referral forms for prescribers are collected through JotForm's HIPAA-compliant platform (see Section 3).
Information collected automatically
When you visit this website, our servers and third-party analytics tools may automatically collect technical information including your IP address, browser type, operating system, referring URLs, pages visited, and the date and time of your visit. This information is used in aggregate form to understand how the site is used and to improve its performance.

When you submit a form, your data is transmitted directly to and stored within JotForm's secure infrastructure. It is not stored on this website's server or in its database at any point.
Notice of Privacy Practices
As a pharmacy, we are required by HIPAA to maintain a Notice of Privacy Practices (NPP) that describes in detail how we use and disclose your health information in the course of providing care. A copy of our NPP is available upon request by calling 1-855-222-6730 or visiting our pharmacy at 2752 Enterprise Rd, Suite B, Orange City, FL 32763.
This website's Privacy Policy does not replace or supersede the NPP. If there is any conflict between this policy and the NPP as it relates to health information, the NPP governs.

Submissions are never stored on this website's server or in its database. See JotForm's Privacy Policy.
- WordPress: This website is built on the WordPress platform. No form submissions are stored in the WordPress database. General contact inquiries routed through this site are handled via JotForm as described above.
We are not responsible for the privacy practices of external websites linked from this site. We encourage you to review the privacy policies of any third-party sites you visit.
6. Data Retention
Because all forms on this website are processed and stored by JotForm, Accord does not hold form submission data on its own servers. Retention of that data is governed by JotForm's data retention policies and the account settings configured for our forms. If you would like to request deletion of a submission, contact us using the information in Section 10 and we will coordinate that request with JotForm.
Clinical and prescription records created as part of providing pharmacy services are retained within Accord's internal clinical systems, separate from this website, in accordance with applicable Florida Board of Pharmacy rules and federal law.
Analytics data collected through this website is retained in aggregated, anonymized form. Standard server access logs (IP address, request timestamps) are retained for a limited period by our hosting provider for security and troubleshooting purposes.
7. Your Rights
Access and correction
You may request access to the personal information we hold about you as a website visitor by contacting us using the information in Section 10. If any information is inaccurate, we will correct it upon verification.
Health information rights
Your rights regarding health information (PHI) held by Accord Specialty Pharmacy as your pharmacy are described in our HIPAA Notice of Privacy Practices, available upon request. These rights include the right to access your records, request amendments, and obtain an accounting of disclosures.
California residents
If you are a California resident, you may have additional rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information is collected, the right to request deletion, and the right to opt out of the sale of personal information. Do Not Track
Some browsers transmit Do Not Track (DNT) signals. Because there is no universally accepted standard for responding to DNT signals, this website does not alter its data collection practices in response to them.
8. Children's Privacy
This website is not directed to children under the age of 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided us with personal information, please contact us and we will promptly delete it.
If a minor is a patient of Accord Specialty Pharmacy, their health information is handled under the direction of a parent or legal guardian in accordance with applicable law and our HIPAA Notice of Privacy Practices.
